Scheduling Your First Appointment
If you are requesting an appointment and have not been previously treated at the Meridian Group, have not been here for more than 12 months, or have changed insurance plans, please complete the following form. You will be contacted by email or telephone within two business days to let you know what is available. All information is confidential. If you have any questions, call our office at 804-751-8644.
If you are a new patient and have scheduled your first appointment, we would like for you to fill out the appropriate form(s) before you come to the office. Please note that there will be some additional policy forms to read and sign when you come to the office, so you should still arrive early for your first appointment.
